Do you accept insurance?

We do not bill services directly to insurance, but we accept payment from insurance providers. The client is expected to pay in full at the time of service regardless of any arrangement they have made with their insurance provider.

Clients are sometimes successful in obtaining reimbursement for our services from their insurance, but all such reimbursement must be arranged directly between the client and their insurance carrier. We can help provide the necessary documentation to file a claim.

Please check with your insurance company prior to scheduling an evaluation to determine benefits and whether pre-authorization is necessary for psychological testing.

How do I apply for financial assistance?

Please request a financial assistance application with your intake packet. If you have questions about how to complete the form, please call our director of operations or intake coordinator at (208) 333-0008.

Return this application with a copy of the first two pages of your most recent federal or state tax return at your first scheduled meeting at LPLC.

After I submit a financial assistance application, what happens next?

Once we receive your application, the information is reviewed by the finance team. After review, you will receive a letter indicating the approved financial assistance award.
